The obvious thing to double check is that you have configured WebConnect and EmailToCase with the right user name and password. Some of our customers have a doamin policy that forces passwords to be changed every X days and they forget that they must change the configuration in our products every X days as well to match the new password. Recently we found that when trying to submit this form, we get an error:&lt;br&gt;&lt;br&gt;Server Application Unavailable &lt;br&gt;The web application you are attempting to access on this web server is currently unavailable.  Please hit the "Refresh" button in your web browser to retry your request. &lt;br&gt;&lt;br&gt;Administrator Note: An error message detailing the cause of this specific request failure can be found in the application event log of the web server. Please review this log entry to discover what caused this error to occur. &lt;br&gt;&lt;br&gt;I can't find anything in any logs that appears to correspond to a test submit of the form.
